Maintenance Alert

When you find a location or facility needing attention or repair or the safety of others is a concern, please notify the facilities and grounds coordinator or assistant by completing and submitting the form below.

• Please be as detailed and specific as possible in describing the issue. Our goal is to keep our facilities and grounds in the best and safest condition possible.

In an emergency, do not use the online maintenance form.

• First call the parish office during regular business hours at 812-357-5533.
• After hours, contact the facilities and grounds coordinator or assistant.
• When you are not able to speak diectly with a staff member, please contact the chair of the facilities and grounds commission or any member of the commission.
